We would be grateful if you could go to the Medical History form linked below and complete as far as you are able to:
This form is hosted on a secure Google server, information you provide is easy to add and entirely confidential.
Understanding a patient’s medical and dental history increases awareness of existing conditions, or current medication.
Considering these in relation to dental care matters, for safety and good treatment. Your dentist will discuss any points which arise in as much detail as you wish.
In the current situation, we also need to ask our patients to complete a Coronavirus Screening Form. This is not intended to bar anyone, simply to ensure they are treated when safe for them, other patients and staff:
This form is again on a secure Google server, as with any medical information, the contents are treated under strict privacy rules.
Your Dental Wish List
Alongside the medical history form, we also hope patients can take a little time to complete a dental wish list. Having your personal view on what you want to achieve through dental support is valuable:
A dental wish list form can be found at the link below, for you to complete as far as you feel will help:
This form is on the same secure facility, quite straightforward to complete and treated as personal data.
Using Online Forms
Please don’t worry if a few details are not available, these can be added at any stage. Our main objective is to save you time manually filling in medical forms at the practice, when we want you to be relaxed.
We recently moved from PDFs to online forms, at the request of our patients and to save them editing, printing, perhaps posting. The online option should suit most people, although we appreciate there are exceptions.
Should you require any advice on completing the forms, please get in touch. Equally, if you can not complete a form for any reason, this is not an issue and we will record your information at the practice.
