Understanding a patient’s medical and dental history increases awareness of existing conditions, or current medication.
Considering these in relation to dental care is important, for safety and to ensure good treatment decisions. Your dentist will naturally discuss any points which arise in as much detail as you wish.
Your Dental Wish List
Alongside the medical history form, we also hope patients can take a little time to complete a dental wish list. Having your personal view on what you want to achieve through dental support is valuable:

Of the formats available, we find that PDFs are quite universal, regardless of how you access the web. If you do have any problems, let us know, you are welcome to collect blank forms from the practice.
Returning The Form
If you can return the form by email to firstname.lastname@example.org, we will print this out and ask you to sign when you attend for your examination.
Please don’t worry if a few details are not available, these can be added at any stage. Our main objective is to save you time manually filling in medical forms at the practice, when we want you to be relaxed.
Should you require any advice on completing the forms, please get in touch. Equally, if you can not complete a form for any reason, this is not an issue and we will record your information at the practice.
